Abortion in Urban Centres, Small Cities, and Rural Areas: Contrasts in Provision and Accessibility
In 2024, there were 260,000 induced abortions in France, that is, 8,000 more elective abortions and 700 fewer therapeutic abortions than in 2023. Abortions are increasingly provided using medication (80%) and in private practice (44%). In 2024, for the first time, midwives were responsible for the majority of abortions provided through private practice. In large urban centres, the available options are relatively diverse in terms of method, place, and category of healthcare professionals. This is not always the case elsewhere, particularly in rural areas. The increase in the number of midwives authorized to provide medical abortions is, however, expanding access in rural areas. There are more midwives in private practice providing abortion care in rural areas than physicians, and their practice tends to be closer to women’s places of residence. Healthcare facilities (hospitals and clinics) remain essential, because they are the sites where abortions after 9 or more weeks of amenorrhea are performed. However, the geographical distribution of institutions that offer surgical abortions is unequal, in particular for abortions close to the legal time limit or under local anaesthesia, and longer travel times are required to access them. While abortion can be performed anywhere, the range of locally accessible options in terms of methods, places and professions varies widely.
